Submandibular Salivary Gland Resection

A meticulous surgical removal of the submandibular gland to resolve chronic inflammation, stones, or tumors while preserving surrounding nerves and maintaining natural neck contours.

A submandibular salivary gland resection is a specialized surgical procedure that involves removing one of the paired submandibular glands located beneath your jawbone on either side of the neck. At our practice, we understand that procedures involving your neck and throat area can create anxiety. That's why we're committed to providing gentle, expert care. This precise procedure effectively addresses conditions like chronic sialadenitis, sialolithiasis (salivary stones), and benign or malignant tumors affecting the submandibular gland. We perform submandibular gland resections using nerve-monitoring technology and strategic approach techniques that protect the vital marginal mandibular nerve and lingual nerve, resulting in improved functional outcomes and incisions strategically placed within natural neck creases for optimal cosmetic results.

Indications & Symptoms

You may need a submandibular gland resection if you experience:

  • Recurrent swelling or pain under the jaw, especially during or after meals

  • Diagnosed sialolithiasis (salivary stones) that cannot be managed with less invasive methods

  • Chronic sialadenitis (inflammation of the gland) unresponsive to antibiotics

  • A palpable mass or tumor within the submandibular gland

  • Abnormal findings on ultrasound, CT, or MRI of the submandibular region

  • Xerostomia (dry mouth) related to submandibular gland dysfunction

  • Difficulty or pain with swallowing related to submandibular inflammation

  • Recurrent submandibular gland infections

  • Ranula (cystic lesion) originating from the submandibular gland

  • Persistent neck discomfort that worsens with certain foods or during meals

  • Visible swelling in the upper neck under-jaw area

  • Sialorrhea (excessive saliva) unresponsive to other treatments

Treatment Approach

Our approach to submandibular gland resection combines precision with personalized care. The procedure begins with careful patient positioning and appropriate incision planning to maximize access while minimizing visible scarring. Our surgeon's expertise in identifying and preserving critical structures is extensive, particularly the marginal mandibular branch of the facial nerve and the hypoglossal and lingual nerves that are essential for normal tongue movement and sensation. We employ advanced bipolar cautery techniques to minimize bleeding and tissue damage while precisely identifying the gland's vascular pedicle. Our distinctive approach includes preoperative mapping with high-resolution ultrasound when needed, intraoperative nerve monitoring, and meticulous attention to preserving the natural contour of the neck without depression or asymmetry, ensuring both functional excellence and aesthetic satisfaction.

Recovery Guidance

Immediate Post-Procedure (24-72 hours)

  • Keep your head and neck elevated with 2-3 pillows to minimize swelling

  • Apply cold compresses to the surgical area (20 minutes on, 20 minutes off)

  • Take prescribed pain medication as directed to manage discomfort

  • Maintain a soft diet to minimize chewing and reduce strain on the surgical area

  • Keep the incision site clean and dry according to post-operative instructions

  • Watch for any signs of nerve weakness (asymmetrical smile, tongue deviation)

  • Expect some numbness in the surgical area, which is typically temporary

  • Limit neck movement and avoid strenuous activities

  • Continue normal oral hygiene with caution around the surgical site

Short-Term Recovery (1-3 weeks)

  • Attend your follow-up appointment for suture removal (typically 5-7 days after surgery)

  • Continue keeping the incision site clean, applying ointment if recommended

  • Resume normal diet as comfort allows, staying well-hydrated

  • Begin gentle range of motion exercises for the neck as advised by your surgeon

  • Return to work based on your surgeon's recommendation (typically 7-10 days)

  • Use scar management techniques as recommended after sutures are removed

  • Transition to over-the-counter pain relievers as discomfort decreases

  • Resume light activities gradually, avoiding strenuous exercise for 2 weeks

  • Be aware that mild swelling and tightness in the neck will gradually improve

Long-Term Expectations

  • Well-healed incision that typically fades to a thin line within 6-12 months

  • Complete resolution of symptoms related to the diseased submandibular gland

  • Normal salivary function maintained by remaining salivary glands

  • Return of normal sensation to the surgical area, though some may have small areas of permanent numbness

  • No significant change in taste or oral moisture for most patients

  • Follow any additional treatment recommendations based on pathology results

  • Most patients can resume all normal activities without restrictions after 2-3 weeks

  • Annual check-ups may be recommended depending on your diagnosis

Potential Risks

Common Side Effects

  • Temporary numbness around the incision site and lower jaw area

  • Mild swelling and bruising that resolves over 1-2 weeks

  • Minimally visible incision line that typically fades over time

  • Temporary tightness or discomfort with certain neck movements

  • Slight change in salivary volume (typically unnoticeable)

  • Temporary altered sensation in the floor of the mouth

Less Common Complications

  • Marginal mandibular nerve weakness causing temporary asymmetrical smile (usually resolves within weeks)

  • Lingual nerve injury affecting tongue sensation or movement (rare, less than 1% of cases)

  • Hypoglossal nerve injury affecting tongue movement (very rare)

  • Hematoma formation requiring drainage

  • Surgical site infection (rare, occurring in less than 1% of cases)

  • Sialocele (collection of saliva under the skin)

  • Ranula formation (cystic lesion) in the floor of the mouth

  • First bite syndrome (pain with the first bite of a meal)

When to Seek Immediate Medical Attention

  • Excessive bleeding or large swelling at the surgical site

  • Severe pain not controlled by prescribed medication

  • Signs of infection (increasing redness, warmth, pus, or fever)

  • Difficulty swallowing or breathing

  • Significant facial asymmetry or new onset of weakness

  • Significant limitation in tongue movement

  • Opening of the incision or wound separation
